Human oversight that survives a busy week

Human-in-the-loop is easy to write into a policy and hard to sustain in operations. The failure mode we see most often is informal override: people approve AI output because rework is expensive, not because they have time to verify it.
Useful oversight is specific. Name the decision, the minimum evidence required before sign-off, and the escalation path when confidence is low. Train for the exception cases your vendor demo did not show.
Validation is not a one-off launch activity. Run short scenarios quarterly—wrong customer segment, stale data, ambiguous instruction—and record what your team actually did, not what the runbook says they should do.
